In this role, you will be responsible for providing comprehensive case management services to patients in the Atlantis, FL area. As a Registered Nurse / Licensed Clinical Social Worker, you will work closely with patients and their families to assess their needs, develop care plans, and coordinate services to ensure the best possible outcomes. You will also collaborate with healthcare providers and community resources to advocate for the physical, emotional, and social well-being of our patients.
